Do no harm: Who should bear the costs of retired NFL players’ medical bills?

Now there should be an insurance program funded by the league and the players ( for current players). If players opt out of the cost, they have no coverage. With the former players, I have no idea. They didn't plan ahead, nobody planned ahead for them.
