At what point is execution a coaching problem?

Obviously that’s execution, but at what point are these repeated screw-ups by skill position players on the coaches? How are WRs still ending up in the same spot so much? How are we still seeing routes run improperly?

We can’t block any screens, it’s been a problem all season. On first glance at that play, it looks like Hill blocked the wrong player because Johnson is looking to block the same guy, and Hill is further outside but blocks the inside defender. I wonder if Thomas was supposed to be lined up in either position, he left with an injury before this play.
