One of the biggest flaws I see on the Offensive side of the ball

Though I see your point, in real time it appears right decision, but two things I noticed.

Penning did eventually establish his anchor, Carr panicked prior to Penning stopping the defenders momentum. He could have stepped up into the pocket. Penning had the defender locked up as far as grip strength
I don't think Carr should have thrown it away like that, and as you and @Steve in Colorado pointed out there was still room to squeeze through. That said, I've given up on expecting that from Carr. He generally needs a lot of room to operate. Maybe if Penning doesn't get pushed back so far, Carr moves forward and dumps it off to Kamara for a few yards.
