No it hasn't been done yet.. but that doesn't mean it can't be done.

Andy Reid, Dick Vermeil, & Bill Parcells have been to the Super Bowl with multiple teams...

Because McCarthy can't out coach better coaches and its become a trend over the last 7 years. Reid, Vermeil, Parcells.... they all consistently contended with good teams/coaches. McCarthy doesn't. He just gets spanked by em for about the last 7 years.
