I think it should be a divisional opponent on a rotational basis. For instance one year the Saints play a 3rd game against Atlanta. Then the following year play a 3rd contest against Carolina, Tampa, then back to Atlanta.

Also rotate road/home games. For example, if you played Atlanta on the road then on the next 3rd face off against them you get a home game. Since your first 17th game was a road game the next 17th game the following year will be a home contest.

This will work well. One it keeps the schedule clean and simple. Two, it garuantees that the final game each year for every team will be a divisional one. Hence the game is likely to be meaningful and a high interest one. Best of all it will eliminate a Ram vs Jaguars game the final week which has a high likelihood of being meaningless.
 
Play the same place finisher from the other Conference that you played the year before. The NFC South played the AFC South in 2019. In 2020 the 17th game match-ups would be:

Saints vs Texans
Falcons vs Titans
Bucs vs Colts
Panthers vs Jaguars

This or a slight variation where it's from two years prior, so as to spread inter conference matchups between the current rotation. For example, we played the AFC north two years ago, so same place finisher would match us with the Ravens, who we'd then see again two years later as part of the regular rotation

Right, and all the AFC teams would play at home in odd years (so each division has the same number of home games) and NFC is home in the even years.
 
Is it set in stone that their will be a 17th game and when will this start?
Yes. Per the CBA which was voted on and approved this weekend by the NFLPA. The 17th game won’t start during the 2020 season but 2021 at the earliest.
