I always felt that the Thanos we saw at the end of the film wasn't the "actual" Thanos. I felt that the idea that he would "sit and watch the sun rise over a grateful universe" was not literal. That would be too damn cheesy it seems and not very "Russo". He took way too much damage to his body to walk away unscathed like that, even with the Infinity Stones IMO. I feel that there is something else at work there. Perhaps in some suspended state, alternate reality or realm, dram state, etc.
If he used the time stone to reverse the damage from the axe then it would have reversed the damaged to the gauntlet and himself as well.

There's a theory that since Dr. Strange could see the timeline/possible future where Thanos is defeated, he must not be all the way dead. The Ancient One said that “I’ve spent so many years peering through time, looking at this exact moment, but I can’t see past it. I’ve prevented countless, terrible futures. And after each one there’s always another. And they all lead here…” and that she could never see past the moment of her own death. Prediction.. He managed to transport himself to the Quantum realm just as the snap occurred. He is weakened, unable to return, and Scott finds him. They use the time thingies in the Q realm to return and find help for Dr. Strange (Ancient One?) Then it's save the universe as usual.
