It probably depends on your insurance carrier, but I would check with your primary care physician to see if that is something they will refer. Insurance companies tend to have more leeway when a procedure is requested by the PCP or a specialist. Once you know that, give the insurance company a call and they can look up your specific benefits to give you an accurate assessment of what's covered.

I think I had my first one at 30 and it was covered by insurance, so they will do them early when needed. I needed it and am so glad that I had it. They found polyps that they caught early and were able to remove.

Probably depends on insurance, but it's 50 for no history and 40 if there is history from a quick google search. Definitely not medical advice. Honestly, talk to your Dr.

With no history the new suggestion is 45. So, if you talked to your doctor they could probably write up a referral for some
Reason or another. Got any bad habits that might raise the risk?
